Over the past few years, the Internet has dramatically changed how healthcare professionals interact with patients and each other and how they access clinical information. The Internet can be great, but also presents challenges. Crawling pages of search engine results, wondering about the integrity of the sources can be daunting to anyone looking to quickly find reliable information. Epocrates partnered with BMJ Group, publisher of the British Medical Journal, to create this new web-based reference which delivers practical and up-to-date disease diagnosis and treatment guidelines at the immediate point of care. The product features peer-reviewed content that is authored by physician experts with guidance by BMJ Group. With the new disease database, you’ll get condition overview and risk factors, treatment approach, tests to order, national medical society guidelines, and more.

As computers become a staple in exam rooms, access to references such as Epocrates Online will become more prevalent too, making clinicians even more connected. Are you prepared to share these resources with your patients? Consider the following tips from internist and Epocrates’ senior medical editor, Dr. Anne Meneghetti:

  1. Arrange monitors and devices to minimize physical and perceived barriers between you and patients
  2. Communicate the purpose for using electronics (“I’m checking your insurance coverage”)
  3. Invite patients to directly view displayed information that might give insight into their condition or care; print out available patient education or drug/disease information, or offer to email a link
  4. Ask patients to “brown bag it” once a year, filling a bag with current prescription and OTC medications, herbs and supplements to identify any potential drug interactions

Most of us decided whether we were destined for academia or athleticism long before the close of our second decade. There are notable exceptions, including Greek physician-athlete Gregoris Lambrakis (1912-1963), who garnered the gold for long jump in the Balkan Games, then ran a private clinic for underserved patients.

The British Medical Journal recently reported results from a prospective cohort study1 of 8762 men, exploring the relationship between muscle strength and all-cause mortality. Muscle strength was quantified by leg and bench press, cardiovascular fitness by exercise treadmill. Around 19 years later, after adjusting for age, CV fitness, activity, smoking, alcohol, BMI, PMHx, and FHx CV disease, muscle strength was inversely and independently associated with death from all causes and cancer. Despite the inherent limitations in a study of this design, these results highlight the importance of muscular strength, and provide encouragement for physicians and patients alike to get strong.

This prospective cohort study followed 8762 men aged 20-80. Muscle strength was quantified by combining one rep. maximal leg and bench presses; CV fitness was assessed by a max. treadmill exercise test. After around 19 years follow-up, 503 deaths occurred (145 CV disease, 199 cancer). Adjustments were made for age, physical activity, smoking, alcohol intake, BMI, baseline medical conditions, FHx CV disease.

“I am a medical student at the Moi University School of Medicine located in Eldoret, Kenya. I am trying to read and catch up with my academics, working hard to hide my excitement for the Beijing 2008 Olympic Games. My favorite athlete is Kosgei Mercy Jelimo, the 800 meters world junior champion. With numerous titles behind her, she is the normal girl that I meet in my town. While exercise has been emphasized in many developed countries to reduce risk of cardiovascular disease, the high malnutrition rate in Kenya will not allow for obesity. The long distance that we had to walk to school and the heavy corporal punishment for any lateness are a great contributor to the talent identified for long distance running in Kenya. However the winning has been great for peace efforts in my country. The athletes have been at the forefront in organizing for intercommunity races. Kenya is just recovering from the worst post election violence that was ethnically based, but for the under 2 minutes that we will cheer Jelimo on to get a new world record, all the 52 tribes in Kenya will be one…”
