Adult Dosing .

Dosage forms:  TAB: 0.18 mg

opioid withdrawal

[3-4 tabs PO q5-6h for up to 14 days]

Start: 3 tabs PO q5-6h during peak withdrawal sx period; Max: 4 tabs/dose up to 16 tabs/day; Info: adjust dose based on clinical response and adverse effects; consider decr. dose to 1-2 tabs PO q5-6h in pts 65 yo and older; taper dose gradually over 2-4 days to D/C

renal dosing

eGFR 30-89.9: 2 tabs q5-6h; eGFR <30: 1 tab q5-6h

HD/PD: 1 tab q5-6h; no supplement
