Invasive group A streptococcal infections surge in 10 U.S. states

Study details: This study analyzed trends in invasive Group A Streptococcal (iGAS) infections across ten U.S. states from 2013 to 2022. Data were collected from the Active Bacterial Core surveillance network, covering 34.9 million individuals. Cases were defined by the isolation of GAS from sterile sites or wounds in patients with necrotizing fasciitis or streptococcal toxic shock syndrome.
Results: The study identified 21,312 cases of iGAS, including 1,981 deaths. Incidence rates rose from 3.6 per 100,000 in 2013 to 8.2 per 100,000 in 2022. The majority of cases (57.5%) were in males, with the highest incidence among those aged 65 and older. However, the greatest relative increase was seen in adults aged 18 to 64 years. Incidence was also higher among American Indian or Alaska Native persons. Notably, macrolide and clindamycin nonsusceptibility increased from 12.7% to 33.1%.
Clinical impact: Enhanced prevention and control measures are needed for iGAS, particularly among high-risk groups such as older adults, American Indian or Alaska Native persons, and those with underlying conditions. Early diagnosis and targeted treatment are essential to reduce morbidity and mortality.
