Safety/Monitoring .
Monitoring Parameters
GIST: pregnancy test at baseline; signs/symptoms of intracranial hemorrhage
Advanced Systemic Mastocytosis: pregnancy test at baseline; Plt at baseline, q2wk x8wk, then q2wk or more frequently as clinically indicated if Plt <75,000, q4wk if Plt 75,000-100,000, or as clinically indicated if Plt >100,000; signs/symptoms of intracranial hemorrhage
Indolent Systemic Mastocytosis: pregnancy test at baseline; signs/symptoms of intracranial hemorrhage
Pregnancy/Lactation .
Pregnancy
Clinical Summary
weigh risk/benefit during pregnancy; no human data available; risk of teratogenicity, embryo-fetal toxicity and death, and decr. fetal wt based on animal data at 2.7x or greater systemic exposure and drug's mechanism of action
Individuals of Reproductive Potential
obtain pregnancy test before tx start; avoid pregnancy by using effective contraception during tx and x6wk after D/C in patients of childbearing potential and male patients
Lactation
avoid breastfeeding during tx and x2wk after D/C; no human data available to assess risk of infant harm or effects on milk production
Pharmacology .
Metabolism: for avapritinib: oxidation, conjugation; CYP450: 2C9, 3A4/5 (primary) substrate; UGT: 1A3 substrate
Excretion: for avapritinib: feces 70% (11% unchanged), urine 18% (0.23% unchanged); Half-life: 32-57h (GIST), 20-39h (advanced systemic mastocytosis), 38-45h (indolent systemic mastocytosis)
Subclass: Kinase Inhibitors, KIT Inhibitors ; Kinase Inhibitors, PDGFRA inhibitors
Mechanism of Action
for avapritinib: inhibits multiple tyrosine kinases including KIT D816V, PDGFRA, PDGFRA D842 mutants, multiple KIT exon 11, 11/17 and 17 mutants, interfering with tumor and mast cell proliferation
