diphtheria, tetanus, pertussis, poliomyelitis, hepatitis B infection and invasive Haemophilus influenzae type b disease prevention

- Dose: 0.5 mL IM x1 at 2 mo, 4 mo, and 6 mo for 3 total doses; Info: may give 2nd dose when at least 10 wk old; may give 3rd dose when at least 24 wk old; may be used to complete hepatitis B and Hib immunization series, additional vaccines needed to complete DTaP and IPV immunization series; give booster x1 with single agent HBV vaccine if anti-HBs level <10 milliunits/mL at least 1-2mo after last dose in patients at least 9 mo with HBsAg-positive gestational parent or HBsAg status unknown gestational parent; search 'immunization' for epocrates Child/Adolescent Immunization decision tool including links to CDC/ACIP schedules; see pkg insert for schedule for patients previously vaccinated with 1 or more components
