Dosage forms:  PATCH: 5 mg per day, 10 mg per day

Alzheimer dz, mild-severe dementia

Start: apply 5 mg patch qwk x4-6wk, then may incr. to 10 mg patch qwk; Max: 10 mg/day qwk; Info: do not cut/alter patch; if switching from PO, apply patch w/ last PO dose; if switching from 5 mg PO qhs for at least 4-6wk or 10 mg PO qhs, then may start 10 mg patch qwk
