dexrazoxane

Adult Dosing .

Dosage forms:  INJ

cardiomyopathy prophylaxis, doxorubicin-assoc.

[10:1 dexrazoxane:doxorubin ratio IV x1 before each doxorubicin dose]

Info: for patients w/ metastatic breast CA who have received cumulative doxorubicin dose of 300 mg/m^2 and are continuing doxorubicin tx; give doxorubicin w/in 30min after completing dexrazoxane dose

extravasation tx, IV anthracycline-assoc.

[1000 mg/m^2/dose IV q24h x2 on days 1-2, then 500 mg/m^2/dose IV x1 on day 3]

Start: ASAP w/in 6h of extravasation; Max: 2000 mg/dose on days 1-2, 1000 mg/dose on day 3

renal dosing

CrCl <40: decr. usual dose by 50%

hepatic dosing

hepatic impairment: decr. usual dose proportionally to reduction in doxorubicin dose, maintaining 10:1 ratio
