Adult Dosing .

Dosage forms:  CHEWABLE: 500 mg, 750 mg, 1000 mg; POWDER: 750 mg per packet, 1000 mg per packet

hyperphosphatemia, ESRD

[1500-3000 mg/day PO divided tid]

Start: 500 mg PO tid; Max: 4500 mg/day; Info: may titrate dose by 750 mg/day q2-3wk to target serum PO4 levels; give with or immediately after meals; chew completely or crush tab before swallowing; consider D/C if unexplained severe GI symptoms
