12 yo and older

Dose: 1 tab PO qd-bid prn; Max: 2 tabs/24h; Info: give up to 30min before meals for s/sx prevention

CrCl 20-40: decr. usual dose by 25%; CrCl <20: decr. usual dose by 50%

HD: give usual dose q12h prn, on dialysis days admin. after dialysis; no supplement; PD: give usual dose q12h prn; no supplement

CrCl 10-50: decr. usual dose by 50%; CrCl <10: decr. usual dose by 75%
