Dose: load (units) = (100-baseline AT activity)/2.3 x actual body wt (kg) IV x1, then maintenance (units/h) = (100-baseline AT activity)/10.2 x actual body wt (kg) IV infusion; Start: 24h preop; Info: individualize dose to maintain AT activity levels between 0.8-1.2 units/mL; see pkg insert for dose adjustments; continue tx until adequate anticoagulation established

[pregnant patients]

Dose: load (units) = (100-baseline AT activity)/1.3 x actual body wt (kg) IV x1, then maintenance (units/h) = (100-baseline AT activity)/5.4 x actual body wt (kg) IV infusion; Start: immediately prior to delivery; Info: individualize dose to maintain AT activity levels between 0.8-1.2 units/mL; see pkg insert for dose adjustments; continue tx until adequate anticoagulation established
