HD: start 3 mg x1 on days 1, 8, 15 of 28-day cycle; no supplement after dialysis; PD: start 3 mg x1 on days 1, 8, 15 of 28-day cycle; no supplement

hepatic dosing

bilirubin >1.5x ULN: start 3 mg x1 on days 1, 8, 15 of 28-day cycle
