Amoxicillin-clavulanate vs. amoxicillin for pediatric acute sinusitis

Among children with acute sinusitis, there was no difference in treatment failure risk between patients who received amoxicillin-clavulanate vs. amoxicillin, but amoxicillin-clavulanate was associated with an increased risk of GI symptoms and yeast infections.

  • This cohort study included included 320,141 children and adolescents ages 17 years or younger with a new diagnosis of acute sinusitis and a same-day new prescription of amoxicillin-clavulanate or amoxicillin.
  • Treatment failure occurred in 1.7% overall, and 0.01% had serious failure (i.e., an ED or inpatient encounter). There was no difference in the risk of treatment failure between the amoxicillin-clavulanate and amoxicillin groups (relative risk, 0.98; 95% confidence interval, 0.92-1.05).
  • These findings may help inform clinical decisions for selecting antibiotics in acute sinusitis.
