Pharmacist-led outreach improves regimen safety in high‑risk type 2 diabetes

A randomized trial (NCT06746714) of 191 adults with type 2 diabetes at high risk of hypoglycemia found that proactive outreach from a clinical pharmacist using an evidence‑based deprescribing algorithm increased transitions to safer regimens (28% vs. 16% with usual care). Patients in the intervention arm also had fewer hypoglycemia‑related ED or inpatient encounters (0 vs. 5) without worsening HbA1c control. The authors note that embedding protocol‑driven pharmacist review into team‑based care may help limit iatrogenic hypoglycemia risk.
Clinical takeaway: Consider structured pharmacist‑supported deprescribing workflows to reduce hypoglycemia‑prone therapies without compromising glycemic control.
