Telemedicine now accounts for nearly 1 in 5 PrEP prescriptions in the US

A retrospective cohort study of 162,422 patients receiving telemedicine-based PrEP (telePrEP) from a national provider found rapid growth in use from 2018 to 2025. TelePrEP accounted for 2% of U.S. PrEP prescriptions in 2020 and 19% in 2024. Most patients were young men (mean age, 33 years), 77% were new to PrEP, and 36% were uninsured. Home lab specimen collection was common (81%). Retention in care remained stable across cohorts, with 67% to 73% retained after one year and about one third after three years. Authors note telePrEP’s scale and reliance on supportive policies like ACA coverage and 340B pricing.
Clinical takeaway: TelePrEP is emerging as a critical component of HIV prevention infrastructure, offering accessible care for uninsured and hard-to-reach populations.
