- Start: 0.5 ng/kg/min IV, then incr. by 0.5 ng/kg/min q30min, x6h/day on day 1-3; Max: 2 ng/kg/min; 8 days; Info: may decr. by 0.5 ng/kg/min q30min if not tolerated; may interrupt tx if dose-limiting adverse reaction and resume after symptom resolution; day 4 onward, start at highest tolerated dose from previous day
