Dose: 0.2 mg/kg/dose IV x1, then 0.1 mg/kg/dose q12-24h x2 doses; Info: hold if urine output <0.6 mL/kg/h; may repeat course x1

[premature neonates, 2-7 days old, 500-1750 g]

Dose: 0.2 mg/kg/dose IV q12-24h x3 doses; Info: hold if urine output <0.6 mL/kg/h; may repeat course x1

[premature neonates, >7 days old, 500-1750 g]

Dose: 0.2 mg/kg/dose IV x1, then 0.25 mg/kg/dose q12-24h x2 doses; Info: hold if urine output <0.6 mL/kg/h; may repeat course x1
