Black Box Warnings .
Nephrotoxicity
dose-related, cumulative, severe renal toxicity including acute renal failure can occur; ensure adequate hydration and monitor renal function, electrolytes; consider decr. dose or alternative tx in patients with renal impairment
Peripheral Neuropathy
may cause dose-related peripheral neuropathy; more severe with repeated tx; can have delayed onset from 3-8wk after cisplatin D/C; may be irreversible; perform neurologic exam before tx start, as clinically indicated during tx and after cisplatin D/C
Nausea and Vomiting
severe cases can occur; premedicate with highly effective antiemetic regimen; may begin within 1-4h after tx and last up to 72h
Myelosuppression
may cause severe myelosuppression with fatal infections; monitor blood counts before tx start, prior to each course and as clinically indicated; may require tx interruption; possible incr. risk in geriatric patients
