Appropriate Use
weigh risk/benefit; use ketoconazole tablets only when other effective antifungal tx not available or tolerated
Hepatotoxicity
serious cases, including fatal or ones requiring liver transplant, have occurred with oral use, even in patients without hepatic disease risk factors; inform patients of risk and monitor closely
QT Prolongation and Drug Interactions
contraindicated with dofetilide, quinidine, pimozide, cisapride, methadone, disopyramide, dronedarone, or ranolazine due to QT prolongation risk; ketoconazole may incr. drug levels and prolong QT interval, resulting in life-threatening ventricular dysrhythmias such as torsades de pointes

Dosage forms: TAB: 200 mg
infections, fungal

- Info: for blastomycosis, coccidioidomycosis, histoplasmosis, chromomycosis, or paracoccidioidomycosis; dose, duration vary with infection type, severity; give with food
prostate CA, castration-resistant (off-label)
- [400 mg PO q8h]
- Info: use as monotherapy or with hydrocortisone; not 1st line tx; give with food
Cushing syndrome (off-label)
- [400-1600 mg/day PO divided bid-qid]
- Start: 200-600 mg/day PO divided bid-tid; Info: adjust dose based on clinical response and urinary free cortisol levels or serum cortisol levels
