C. difficile Associated Diarrhea
mild diarrhea to fatal colitis possible on tx or for >2mo after D/C lincomycin; consider diagnosis if diarrhea presentation after antibiotic administration; antibiotics alter colon flora, leading to C. difficile overgrowth; C. difficile produces toxins A and B which contribute to C. difficile associated diarrhea; hypertoxin producing strains cause incr. morbidity and mortality since these infections can be refractory to antibiotic tx and may require colectomy; patient may need fluid, electrolyte, and protein suppl. along with antibiotic for C. difficile; surgical evaluation as needed; reserve lincomycin for serious infection where less toxic antibiotic inappropriate; avoid use for nonbacterial infection including most URIs
