immune globulin (human) and hyaluronidase (recombinant human)
Black Box Warnings .
Thrombosis
may occur with immune globulin products; risk factors include advanced age, prolonged immobilization, hypercoagulable conditions, venous or arterial thrombosis history, estrogen use, indwelling central vascular catheter, hyperviscosity, and cardiovascular disease risk; thrombosis may occur in patients without risk factors; use minimum dose and infusion rate practicable in patients at risk; ensure adequate hydration before administration; monitor for signs/symptoms of thrombosis and assess blood viscosity in patients with hyperviscosity risk
Adult Dosing .
Dosage forms: INJ (pack): immune globulin 2.5 g per 25 mL vial x1 and hyaluronidase 200 units per 1.25 mL vial x1; INJ (pack): immune globulin 5 g per 50 mL vial x1 and hyaluronidase 400 units per 2.5 mL vial x1; INJ (pack): immune globulin 10 g per 100 mL vial x1 and hyaluronidase 800 units per 5 mL vial x1; INJ (pack): immune globulin 20 g per 200 mL vial x1 and hyaluronidase 1600 units per 10 mL vial x1; INJ (pack): immune globulin 30 g per 300 mL vial x1 and hyaluronidase 2400 units per 15 mL vial x1

- Info: refer to institution protocols and pkg insert prior to prescribing
primary immunodeficiency
- [immune globulin component]
- Dose: 300-600 mg/kg/dose SC infusion q3-4wk; Start: 1wk after receiving last immune globulin dose give 25% previous monthly immune globulin dose SC infusion x1 on wk 1, then give 50% previous monthly immune globulin dose SC infusion x1 on wk 2, then give 75% previous monthly immune globulin dose SC infusion x1 on wk 4, then if required give 100% previous monthly immune globulin dose SC infusion x1 on wk 7; Alt: maintain previous IVIG dose and frequency via SC infusion starting on wk 7 if switching from other IVIG; Info: give 10min after hyaluronidase component; adjust immune globulin dose based on trough IgG levels and clinical response; give 400 mg/kg/dose IV x1 ASAP within 6 days if measles exposure; incr. maintenance dose to at least 530 mg/kg/dose IV q3-4wk if measles exposure risk
- [hyaluronidase component]
- Dose: 80 units per g immune globulin SC infusion with each immune globulin dose; Info: give before immune globulin component; give full vial hyaluronidase per each full or partial vial immune globulin
chronic inflammatory demyelinating polyneuropathy
- [immune globulin component]
- Dose: maintain previous IVIG dose and frequency via SC infusion q2-4wk; Start: 2wk after receiving last immune globulin dose give 25% previous monthly immune globulin dose SC infusion x1 on wk 2 and 3, then give 50% previous monthly immune globulin dose SC infusion x1 on wk 4, then give 75% previous monthly immune globulin dose SC infusion x1 on wk 6, then give 100% previous monthly immune globulin dose SC infusion x1 on wk 9; Max: 1200 mL if wt >40 kg; 600 mL if wt <40 kg; Info: for patients stable on IVIG; give within 10min after hyaluronidase component; adjust immune globulin dose based on clinical response
- [hyaluronidase component]
- Dose: 80 units per g immune globulin SC infusion with each immune globulin dose; Info: give before immune globulin component; give full vial hyaluronidase per each full or partial vial immune globulin
