avoid combo if antitussive use; otherwise, use alternative or monitor respiratory rate, BP; decr. dose of one or both drugs, use lowest effective doses and shortest duration of concomitant tx: combo may incr. codeine and active metabolite morphine levels, risk of profound CNS and respiratory depression, psychomotor impairment, severe hypotension (including orthostasis, syncope), other adverse effects (hepatic metabolism inhibited, may incr. production of active metabolite morphine; additive effects)

diclofenac + acetaminophen

use alternative or monitor LFTs; use lowest effective doses, shortest effective tx duration: combo may incr. risk of hepatotoxicity (possible additive effects)

codeine + ethinyl estradiol (contraceptive)

use alternative or monitor respiratory rate, withdrawal sx: combo may incr. codeine, alter active metabolite morphine levels, incr. risk of CNS and respiratory depression, psychomotor impairment, other adverse effects or decr. efficacy (hepatic metabolism inhibited, may alter production of active metabolite morphine)

acetaminophen + ethinyl estradiol (contraceptive)

monitor thrombosis s/sx: combo may incr. ethinyl estradiol levels, risk of thromboembolism, other adverse effects (mechanism unknown)

flibanserin + ethinyl estradiol (contraceptive)

monitor BP: combo may incr. flibanserin levels, risk of CNS depression, psychomotor impairment, severe hypotension (including syncope), other adverse effects (hepatic metabolism inhibited)

diclofenac + desogestrel (contraceptive)

monitor thrombosis s/sx, especially w/ concomitant estrogen use; use lowest effective diclofenac dose, shortest effective tx duration: combo may incr. risk of thromboembolism (additive effects)

diclofenac + ethinyl estradiol (contraceptive)
