Antioxidant intake tied to lower cervical cancer prevalence

Clinical takeaway: The antioxidant association can enter the conversation around prevention. But it should not shift attention away from screening intervals or HPV vaccination status.
Patients ask what they can eat to lower their cancer risk, and cervical cancer is a hard place to answer well. The dominant cause is a virus, and the interventions with real prevention evidence are a vaccine and a screening test. That leaves diet in an uncomfortable position: plausible, widely believed, and thinly supported. A new analysis of national survey data reports that women with higher dietary antioxidant intake were less likely to report a cervical cancer history, which is worth knowing about mainly because patients will encounter the same claim.
Persistent infection with high-risk HPV is the principal cause of cervical cancer, and vaccination and screening are the established ways to prevent it. But not every infection progresses, which has kept interest alive in what else might modify the course. Diet is one of the potential candidates. Researchers have reported inverse associations between antioxidant intake and high-risk HPV infection, though whether anything similar holds for cervical cancer itself had not been examined directly.
Women in the highest third of dietary antioxidant intake had a 55.8% lower prevalence of reported cervical cancer than those in the lowest third, after adjustment for smoking, body mass index, income, education, marital status, diabetes, and hypertension. Scored as a continuous measure, each one-unit increase in the index tracked with an 11.4% lower prevalence. The middle third did not separate from the bottom third.
The two groups differed in ways that bear on the finding. Women reporting cervical cancer were more likely to smoke, more likely to have obesity, and more likely to live alone, and 39% fell into the lowest income category compared with 26% of controls. The analysis adjusted for those differences, but it did not include HPV status, screening history, or vaccination status, so their potential influence could not be assessed.
The investigators pulled eight NHANES cycles from 2003 to 2018, matching each of the 174 women reporting a cervical cancer history to five controls by age and race. Cervical cancer status came from a single survey question about whether a provider had ever given the diagnosis. Antioxidant intake was scored from two 24-hour dietary recalls.
The antioxidant index was based on dietary intake of vitamins A, C, and E, zinc, selenium, and carotenoids. Prior research has linked these nutrients to immune response, oxidative stress, and inflammation, which are involved in the progression of persistent HPV infection to cervical cancer. Women with cervical cancer have been found to have lower blood levels of several of these nutrients.
"These findings suggest that dietary antioxidant intake may play a potential role in the prevention of cervical cancer. However, further prospective studies are needed to confirm this association and clarify the underlying mechanisms," the authors conclude.
