Blood eosinophil counts: A reliable biomarker for eosinophilic esophagitis management?

This prospective multicenter study of 209 patients found that peripheral blood eosinophil counts accurately distinguished eosinophilic esophagitis (EoE) from non-EoE dysphagia and predicted histological remission. An absolute eosinophil count (AEC) of 155 eosinophils/mm³ achieved 85% accuracy for EoE diagnosis, while an AEC of 325 eosinophils/mm³ showed 70.5% accuracy for identifying histological remission after treatment. Blood eosinophil levels correlated significantly with esophageal tissue eosinophil counts and endoscopic severity scores.
Clinical takeaway: Consider checking blood eosinophil counts as an adjunct to assess EoE activity and support decisions on when endoscopy or treatment modification may be warranted.
