Can air pollution trigger rheumatoid arthritis flares?

Clinical takeaway: For patients with rheumatoid arthritis, consider discussing air quality as part of flare prevention. During periods of sustained poor air quality, reinforcing strategies to reduce outdoor pollution exposure may complement routine disease management.
Rheumatoid arthritis (RA) flares are often difficult to predict, even in patients receiving appropriate therapy. A large prospective cohort study suggests that prolonged exposure to fine-particle air pollution may contribute to worsening disease activity, adding environmental exposure to the growing list of factors that may influence RA control.
Researchers followed 1,070 adults with established RA in South Korea over four years, analyzing 12,583 outpatient visits. Monthly exposure to six common air pollutants was matched with disease activity measures while accounting for demographics, medication use, socioeconomic factors, and weather.
Among all pollutants studied, fine-particle air pollution (PM2.5), which are microscopic particles from sources such as vehicle emissions, wildfire smoke, industrial pollution, and wood burning, showed the strongest and most consistent association with disease worsening. Each standard deviation increase in exposure was associated with an 11% higher odds of RA flare and significantly higher disease activity scores, including more tender and swollen joints.
A sensitivity analysis using daily pollution data suggested that sustained exposure mattered more than short-term spikes. The risk of flare increased after more than two weeks of elevated fine-particle pollution exposure, supporting a cumulative rather than immediate effect. Associations between fine-particle pollution and disease activity were strongest among women and nonsmokers.
“Our study has important implications for public health policy making. While further studies are warranted to determine whether improving air quality can reduce disease activity in RA patients, we would recommend these patients avoid prolonged exposure to poor air quality, particularly high PM2.5 levels,” said lead investigator Eun Bong Lee, MD, PhD, of Seoul National University College of Medicine and Seoul National University.
