Clinical takeaway: Consider HCP-administered tildrakizumab as a new IL-23 inhibitor option for adults with active PsA, particularly for patients who prefer in-office treatment or are unable to self-administer biologic therapy. Screen for TB before initiation and monitor for infections during treatment.

FDA has approved Ilumya (tildrakizumab-asmn) for the treatment of adults with active psoriatic arthritis (PsA), expanding the drug’s indications beyond moderate-to-severe plaque psoriasis.

PsA is a chronic immune-mediated disease characterized by joint pain, swelling, and stiffness that can lead to irreversible joint damage if not adequately controlled. The new indication adds a healthcare provider-administered interleukin-23 (IL-23) inhibitor to the treatment armamentarium, offering an alternative for patients who are unable to self-administer biologic therapy.

Ilumya is a monoclonal antibody that selectively binds the p19 subunit of IL-23 and blocks its interaction with the IL-23 receptor. This reduces inflammatory signaling associated with psoriatic disease.

The approval was supported by the phase 3 INSPIRE-1 and INSPIRE-2 trials. In INSPIRE-1, which enrolled TNF inhibitor-experienced patients, 41% of patients receiving Ilumya achieved an American College of Rheumatology 20% improvement response (ACR20) at week 24 compared with 25% receiving placebo. In INSPIRE-2, which enrolled TNF inhibitor-naïve patients, the corresponding rates were 35% and 23%, respectively.

Safety findings were consistent with Ilumya’s established experience in plaque psoriasis, with no new safety signals identified. Ilumya may increase the risk of infection, and patients should be evaluated for TB before treatment initiation. The most common adverse reactions in PsA were upper respiratory tract infections and COVID-19.

“Psoriatic arthritis is a complex and chronic disease. As clinicians, we need a range of therapeutic options that allow us to individualize care based on each patient’s symptoms and treatment goals. Adding an IL-23 inhibitor option that is administered by a healthcare provider gives us a new way to offer appropriate patients additional support and engagement with their healthcare team,” said Philip Mease, Director of Rheumatology Research at Swedish Medical Center and Providence St. Joseph Health and Clinical Professor at the University of Washington School of Medicine.
