First-ever guidance addresses cardiovascular care in tactical athletes

Clinical takeaway: When evaluating tactical athletes with cardiovascular disease or risk factors, clinicians should assess not only individual health risk but also the cardiovascular demands of the job, mission-critical responsibilities, environmental exposures, and the potential consequences of sudden incapacitation.
ACC/AHA has issued its first comprehensive scientific statement on cardiovascular care for tactical athletes, a population that includes more than 4 million U.S. military personnel, first responders, pilots, astronauts, and others whose jobs involve extreme physical and environmental demands. The statement introduces a new Tactical Clinical Management (TCM) framework that combines evidence-based cardiovascular care with occupation-specific risk assessment.
Unlike competitive athletes, tactical athletes may face heat stress, altitude exposure, heavy equipment loads, combat conditions, diving operations, or aviation duties where a cardiovascular event could endanger teammates, mission success, or public safety. As statement chair Elizabeth H. Dineen, DO, noted, “A cardiovascular event for a firefighter, police officer or military service member can have implications far beyond the individual, including the success of the mission and lives of others.”
Key recommendations include standardized cardiovascular screening before training or service. All tactical athletes should undergo a cardiovascular history and physical exam, and an ECG should be performed in those younger than 40 years who are exposed to substantial cardiovascular strain or whose sudden incapacitation could jeopardize lives or missions. Routine screening echocardiography, stress testing, genetic testing, or ambulatory monitoring is not recommended in asymptomatic individuals with normal initial evaluations.
The statement also introduces a classification system for tactical tasks and "cardiovascular strain amplifiers," such as extreme temperatures, dehydration, altitude, high G-forces, chronic stress, and carrying heavy loads. These factors should be incorporated into fitness-for-duty assessments and decisions about return to duty after cardiovascular events.
Several drug-related considerations are highlighted. For atrial fibrillation and venous thromboembolism, clinicians should balance the need for anticoagulation against occupational risks from collision, trauma, or deployment to austere environments. In channelopathies such as long QT syndrome, medication adherence is emphasized, and drugs that prolong the QT interval, including some antimalarials, should be considered when assessing fitness for duty. The statement also cautions that antihypertensive selection may need to account for occupational demands, as diuretics can worsen heat intolerance and beta-blockers may impair tolerance to high G-forces.
The document recommends that organizations maintain emergency action plans and ensure rapid AED access, noting that screening alone cannot prevent all sudden cardiac arrests. The authors also call for tactical-athlete registries, improved screening strategies, and cardiovascular risk prediction tools tailored to this unique population.
