Flight attendants, pilots rank highest for radiation-cancer deaths

Clinical takeaway: Remember to review patient history for risky work environments, including flight crew work, when assessing cancer risk.
Occupational radiation exposure usually comes with a paper trail. US aircrews are an exception: they absorb more ionizing radiation on the job than any other workforce in the country, but no agency requires that it be measured.
The Federal Aviation Administration (FAA) formally recognizes flight crews as occupationally exposed to ionizing radiation. But it sets no dose limits and requires no monitoring, which puts US crews outside the framework that governs other radiation-exposed categories of US workers and outside the protections aircrew have in Europe. Earlier attempts to connect that exposure to cancer deaths ran into small cohorts and single-site analyses. A new cross-sectional analysis of 12.7 million US death certificates from 2020 to 2024, newly linked to each decedent's usual occupation, compared more than 500 occupations.
Radiation-related cancers made up a risk-adjusted 6.9% of deaths among flight attendants and 6.7% among pilots, the highest and second-highest shares among occupations assessed. The pattern held in both sexes. For cancers not linked to radiation, both groups ranked near the middle of all occupations, which is the contrast that makes the exposure explanation plausible.
The odds of cancer death were elevated across the board. They reached significance for four cancers in both groups: breast, central nervous system, and prostate cancers, plus melanoma, with odds 28% to 75% higher than in all other occupations combined. Pilots also had higher odds of leukemia death. Flight attendants, unlike pilots, showed a modest excess in non-radiation-related cancers as well, which slightly weakens the specificity argument for that group.
The analysis included 14,190 pilot deaths and 7,170 flight attendant deaths, identified by usual occupation on the death certificate, adjusted for age at death, sex, race, ethnicity, education, and marital status. Controls behaved as expected: nuclear technologists ranked 12th, while ground-based aircraft mechanics and assemblers showed no excess deaths.
Aircrew accrue an estimated 3 to 6 millisieverts (mSv) per year depending on flight paths, while a passenger taking 10 cross-country round trips absorbs about 0.4 mSv. That order-of-magnitude gap means the frequent business traveler asking about cancer risk is not the patient this study describes. A career spent in the cabin is the issue, since exposure is cumulative over years of routine flying.
"These findings support consideration of occupational radiation protections for aircrew members commensurate with their exposure level," the authors conclude.
