Glaucoma, cataract risk tied to air pollution levels

Clinical takeaway: Add air quality to the exposure history for older patients, and refer for eye exams sooner when sustained exposure and visual symptoms coincide. Current local PM2.5 levels are available through the EPA's AirNow.gov.

Glaucoma and cataracts are usually explained to patients as diseases of aging. For clinicians, that framing leaves little to work with beyond age, family history, and a handful of clinical contributors. There has been no modifiable exposure to point to when patients ask why their vision is deteriorating. 

Air pollution has long been a candidate. Its harms to the heart and lungs are well established, and the eye is in direct contact with ambient air. But the evidence on eye disease has come from scattered individual studies, too varied in design and population to settle the question. A systematic review and meta-analysis pulls that evidence together; the pattern it finds is consistent enough to matter for how clinicians think about both conditions. 

The strongest and most consistent associations were with fine particulate matter, PM2.5. These particles, from smoke, vehicle exhaust, and industrial pollution, are small enough to penetrate deep into the body. For each interquartile-range increase in exposure, standardized to 10 μg/m³ where applicable, the odds of glaucoma rose 8%, with high-certainty evidence behind the estimate. The gradient held at the extremes: people in the highest exposure quartile had 70% higher odds of glaucoma than those in the lowest, and study-to-study agreement on that comparison was complete. Cataract risk rose 4% per increment. 

Ozone produced the analysis's other high-certainty finding, a 14% higher risk of age-related macular degeneration per exposure increment. The remaining pollutants, larger PM10 particles, nitrogen dioxide, and sulfur dioxide, showed no statistically significant association with any of the three diseases. 

The meta-analysis, presented at the 44th Congress of the European Society of Cataract and Refractive Surgeons (ESCRS), pooled 41 observational studies of adults across East Asia and the UK, spanning millions of people and six ambient pollutants. 

The pooled studies came from East Asia and the UK, not from the regions with the world's heaviest particulate burdens. The researchers say eye health in the most highly polluted regions, such as South Asia and sub-Saharan Africa, is where study is needed next. They also argue the findings belong as part of air quality policy debates that have so far rested almost entirely on cardiovascular and pulmonary harms. For US clinicians, the exposure gradient in these data spans levels found in most cities. 

"Both conditions become more common as we get older, but this study indicates that people who are exposed to higher levels of air pollution are also facing a greater risk," said Filomena Ribeiro, head of the ophthalmology department at Hospital da Luz in Lisbon, Portugal, and immediate past-president of the ESCRS, who was not involved in the research. "This research adds eye health to the growing list of reasons to care about air quality."
