GLP-1s show promise for reducing binge eating symptoms

Clinical takeaway: GLP-1 receptor agonists may eventually become an adjunct to psychological therapy for binge eating disorder in patients with obesity, but larger trials are needed before routine use for this indication.

Binge eating disorder is the most common eating disorder worldwide, yet treatment options remain limited and no medications are currently approved specifically for the condition. A new systematic review and meta-analysis suggests that GLP-1 receptor agonists, widely used for obesity and type 2 diabetes, may reduce core binge eating symptoms in addition to promoting weight loss.

Researchers analyzed 25 randomized controlled trials involving 8,069 participants from 12 countries. Most studies evaluated semaglutide, tirzepatide, or liraglutide in adults with obesity, although relatively few participants had a formal diagnosis of binge eating disorder.

Across studies, GLP-1 receptor agonists significantly reduced binge eating episodes, loss-of-control eating, and emotional eating compared with control treatments. Participants also reported greater dietary restraint, although the investigators noted it remains unclear whether this reflects healthier self-regulation or could signal problematic restrictive eating in some patients.

For patients with obesity who also experience binge eating, GLP-1 receptor agonists may eventually offer dual benefits by reducing both weight and binge eating symptoms. However, the evidence should be interpreted cautiously. Most included trials had a high risk of bias, were industry funded, and rarely enrolled patients with clinically diagnosed binge eating disorder, limiting confidence in applying these findings directly to routine psychiatric practice.

The meta-analysis identifies GLP-1 receptor agonists as a promising area for future research in binge eating disorder, but larger trials are needed to determine whether these short-term benefits translate into sustained clinical improvements.

“We hope that by highlighting the potential of weight loss drugs to help with binge eating symptoms, our findings will lead to further funding of larger high-quality studies in this area, to better understand how this medication could be used in practice and improve treatment options,” said Izzy Emptage, PhD candidate in Psychiatry at University College London and first author of the study.
