More worrisome features raise pancreatic cyst cancer risk

Clinical takeaway: In patients with pancreatic cysts, the number of worrisome features appears to be a key predictor of progression risk. A single worrisome feature was associated with a low 3-year risk of pancreatic cancer or advanced precancerous neoplasia, whereas risk increased substantially in patients with multiple worrisome features, supporting a more individualized surveillance strategy.
More precise risk estimates for pancreatic cysts with worrisome or high-risk features may help clinicians and patients better balance surveillance against surgery, potentially avoiding unnecessary procedures while identifying those most likely to benefit from earlier intervention.
In this prospective cohort study, Mayo Clinic investigators followed 230 patients with pancreatic cysts that had worrisome or high-risk features. Worrisome features, such as cyst size ≥3 cm, moderate pancreatic duct dilation, mural nodules, or rapid cyst growth, are imaging findings associated with an increased risk of progression to pancreatic cancer or advanced precancerous disease.Thirty-two patients underwent surgery shortly after enrollment, while 198 entered surveillance and were followed longitudinally regardless of management strategy.
Risk of progression increased with the number of worrisome features present. Over 3 years, patients with one worrisome feature had an approximately 2% risk of developing pancreatic cancer or advanced precancerous changes. Risk rose to about 4% with two worrisome features and to 18% among those with three or more, identifying a subgroup at substantially higher risk.
Notably, pancreatic cancers detected during surveillance were identified at very early stages, suggesting that carefully selected patients may be safely monitored rather than referred immediately for surgery. The findings provide prospective evidence that risk is not uniform across all cysts categorized as having worrisome features and may support more personalized decision-making.
“This information will help us counsel patients,” said lead author Dr. Shounak Majumder. “If we want to intercept pancreatic cancer while also sparing patients from unnecessary interventions, we must really know what is likely to become cancer and what is not.”
The authors say the results could also help guide future biomarker studies and early-detection strategies aimed at identifying patients most likely to benefit from intervention while reducing overtreatment in lower-risk groups.
