Patients may not need 4 days of dexamethasone for chemotherapy nausea prevention

Clinical takeaway: For patients receiving highly emetogenic chemotherapy with netupitant/palonosetron plus olanzapine, reducing dexamethasone to a single day—or omitting it entirely—may provide similar protection against nausea and vomiting while reducing steroid-related adverse effects. Consider the potential benefits of steroid-sparing approaches, particularly for patients receiving immunotherapy or those at higher risk for corticosteroid toxicity.

Preventing chemotherapy-induced nausea and vomiting (CINV) remains essential for maintaining quality of life and adherence to cancer treatment. However, the standard 4-day dexamethasone regimen adds risks such as hyperglycemia, insomnia, mood changes, and immunosuppression, and there are growing concerns that prolonged corticosteroid use could blunt the effects of immune checkpoint inhibitors.

A phase 3 trial found that patients receiving the antiemetic combination of netupitant/palonosetron plus olanzapine achieved similarly high rates of CINV control whether they received standard 4-day dexamethasone, a single reduced dose on day 1, or no dexamethasone at all.

Complete response—defined as no vomiting and no need for rescue medication during the first 120 hours after chemotherapy—occurred in 72.4% of patients receiving standard dexamethasone, 72.2% receiving a single 6-mg dose on day 1, and 70.1% receiving no dexamethasone. Both steroid-sparing strategies met the study's predefined criteria for noninferiority.

The reduced- and no-dexamethasone regimens also resulted in significantly fewer steroid-related adverse effects than the standard regimen.

The findings are particularly relevant as more patients receive chemotherapy combined with immunotherapy. If confirmed across additional populations and incorporated into treatment guidelines, steroid-sparing antiemetic regimens could help reduce corticosteroid exposure without compromising nausea and vomiting prevention in appropriately selected patients.
