Pharmacist-led management model improves heart failure medication adherence

Study design: This prospective, multicenter randomized trial was conducted from March 2021 to May 2023 in the cardiology wards of five hospitals in China. Participants ≥18 years with a diagnosis of chronic heart failure (CHF) were randomly assigned to either a pharmacist-led management group or a usual care group. The intervention included WeChat-based communication, education, and monthly follow-up visits from a pharmacist, while the control group received monthly follow-up visits from nurses.

Results: Among the 445 participants, those in the intervention group showed significantly higher medication adherence at 52 weeks, with a greater proportion of days covered by heart failure medication and more patients achieving a proportion of days covered of ≥80% compared with the control group.

Impact on clinical practice: Incorporating pharmacists into the management of CHF patients appears to improve medication adherence. Specifically, leveraging social media platforms for communication and education could be a valuable addition to standard care practices, potentially leading to better patient outcomes.
