Physical activity at work tied to higher dementia risk

Clinical takeaway: Occupational activity may not substitute for recreational exercise in lowering dementia risk, and a high weekly total alone is a weak guide to benefit. Consider asking patients about their exercise setting.
Ask a patient whether they get enough physical activity and the answer often depends on their job. Warehouse staff, home health aides, and line cooks are on their feet all day, and activity counseling usually treats that as the box checked. Total volume has been the metric that matters, so a minute spent lifting heavy items on shift has counted the same as a minute spent walking after work.
Activity guidance for brain health has been written from studies of recreational exercise, but that is the minority of how people actually move. Leisure activity accounts for only about 12% of physical activity worldwide; work, household tasks, and commuting account for the rest. For patients in physically demanding jobs, that means the evidence base has never really been about them. A pooled analysis of 74 cohort and case-control studies separated activity by domain and tested each against dementia risk.
Occupational physical activity was associated with a 20% higher risk of all-cause dementia, though the estimate sat close to no effect. Leisure-time activity was tied to a 24% lower risk. Overall, high activity of any kind was associated with 20% lower risk, but most studies feeding that estimate never recorded where the activity happened.
More leisure exercise meant progressively lower risk, but only up to a point. Participants who met standard activity recommendations, 150 to 300 minutes of moderate exercise a week or the vigorous equivalent, showed lower risk than those who did not, and the benefit did not keep growing at higher volumes. Occupational activity worked in reverse: the more activity accumulated on the job, the higher the risk. That pattern held even when the analysis accounted for how much these workers moved overall.
The analysis pooled 74 cohort and case-control studies covering about 4.2 million adults in 30 countries, median age 67 years, followed for a median of 10 years. There were 64,009 dementia cases. Coverage by domain was lopsided: 20 studies measured leisure-time activity against 5 that measured occupational activity, and the authors rated certainty of evidence low to very low throughout. Nearly all relied on patients reporting their own activity levels, and 69 of the 74 studies came from high-income countries.
Physically demanding jobs tend to come with psychosocial stress, low control over the pace of work, exposure to air and noise pollution, and repetitive tasks that build little cognitive reserve. They also leave less time and energy for recreational exercise. The finding is less an argument that work activity is harmful than a signal that the conditions surrounding it are.
"The findings challenge the long-held assumption that 'every move counts' for brain health," said Natan Feter, PhD, postdoctoral fellow in the Evolutionary Biology of Physical Activity Lab at the University of Southern California and first author of the study.
Added senior author David Raichlen, PhD, professor of biological sciences and anthropology at USC: "Improving access to safe parks, walking trails and recreational facilities, ensuring people have adequate leisure time for exercise, and reducing harmful workplace exposures may ultimately prove as important as promoting physical activity itself."
