Retatrutide use from unregulated sources raises safety signals

Clinical takeaway: Ask patients nonjudgmentally about nonapproved weight-loss drugs and peptides obtained online. Counsel about uncertain product quality, assess reported symptoms, and report suspected adverse events to FDA MedWatch.

Patients obtaining investigational weight-loss drugs online may experience symptoms that never reach clinicians or regulators. A new study of Reddit discussions offers a glimpse into this surveillance gap, identifying potential safety signals from self-reported symptoms and effects among people reporting retatrutide use.

Retatrutide targets three hormone receptors involved in appetite and metabolism: GLP-1, GIP, and glucagon. Although it remains investigational, online sellers market products labeled as the drug directly to consumers. FDA warns against purchasing these products, including those labeled “for research purposes” or “not for human consumption,” and states that retatrutide cannot be used in compounding under federal law.

Researchers analyzed approximately 149,000 posts and comments and identified 13,589 users reporting current retatrutide use. After excluding weight-loss and appetite-suppression terms, 7,823 users had at least one remaining symptom or effect report.

Among that subgroup, fatigue was reported by 15.2%, increased energy by 12.4%, nausea by 11.6%, insomnia by 6.1%, and increased heart rate by 4.8%. Increased appetite was the most frequent individual report, despite the drug’s intended weight-loss effects.

Gastrointestinal symptoms were common but did not dominate the discussions. Compared descriptively with earlier Reddit research on semaglutide and tirzepatide, retatrutide reports placed greater emphasis on energy changes, sleep, muscle-related symptoms, and heart rate.

“Nonjudgmental screening for nonapproved peptide use, counseling about unknown product quality and absence of safety data in this population, and reporting of adverse events to MedWatch may improve surveillance and patient safety,” the study authors wrote.
