Shingles shot tied to lower dementia risk in frail seniors

Clinical Takeaway: Consider prioritizing recombinant zoster vaccination (RZV) in eligible older adults, including those with recent skilled-nursing facility stays, as it may confer added protection beyond shingles prevention.

Older adults entering skilled-nursing facilities face elevated risks of both shingles and cognitive decline; a modifiable preventive factor could have major public health impact.

In a target trial emulation using Medicare and electronic health record data from more than 500,000 adults aged 66 and older newly admitted to skilled-nursing facilities, receipt of at least one dose of RZV within 12 months of admission was associated with a significantly lower risk of incident dementia over four years compared with no vaccination.

Dementia developed less frequently among vaccinated patients, translating to a 24% relative risk reduction and an absolute risk reduction of approximately 6 percentage points over four years. The findings were consistent across multiple analyses designed to minimize confounding, although the observational design cannot establish causality.

The authors note that uptake of RZV was relatively low in this high-risk population, highlighting an implementation gap. “Our results suggest that shingles vaccination may have benefits beyond prevention of herpes zoster,” the investigators wrote, while emphasizing the need for further studies to confirm a causal relationship and elucidate mechanisms.

Potential explanations include reduced neuroinflammation or prevention of viral reactivation effects that could contribute to cognitive decline. The study adds to growing evidence linking adult vaccinations with lower dementia risk, but clinicians should interpret the results cautiously pending randomized data.
