Streamlined depression screening: PHQ-3 may offer efficient alternative to PHQ-9

The 3-item PHQ (PHQ-3) provides a highly efficient, reliable measure of depressive symptom severity suitable for large-scale population surveys and settings where minimizing respondent burden is critical. While it cannot fully replace the PHQ-9 in clinical diagnosis, it enables broader, more practical incorporation of depression assessment in epidemiologic research and remote monitoring.

Study details: A large U.S. web-based survey study (N = 96,234 adults; mean age, 47.3 years, 57% women) conducted across four waves (2023–2024) sought to derive a shortened version of the PHQ-9 that would maximize the variance in depressive symptom severity captured. The first wave was used to identify optimal item subsets, and subsequent waves validated the performance of the resulting PHQ-3 across sociodemographic subgroups.

Results: The PHQ-3, comprising items on depressed mood, self-esteem/failure, and loss of interest, demonstrated high internal consistency (Cronbach α = 0.88) and strong correlation with the full PHQ-9 (Pearson r = 0.93). At a threshold score of ≥3, the PHQ-3 showed sensitivity of 0.98 and specificity of 0.76 for detecting moderate or greater depressive symptoms (PHQ-9 ≥10). Performance was consistent across age, gender, race/ethnicity, and education subgroups.
