Sweat patch reads levodopa levels without a blood draw

Clinical takeaway: Continuous levodopa monitoring could eventually let clinicians tailor dose timing to real drug levels instead of patient diaries, though this early device is not yet ready for clinical use.

Clinicians have long known that Parkinson's patients seem to "burn through" levodopa unpredictably, but the pattern has been difficult to pin down without frequent blood draws. A sweat-reading fingertip patch captured it directly: patients cleared the drug significantly faster than healthy subjects at similar bioavailability. The finding puts a mechanism behind the abrupt "off" periods patients describe.

Levodopa remains the backbone of Parkinson's treatment, but keeping it in the therapeutic range gets harder over time. The usable window between too little drug and too much can shrink from several hours to roughly two as the disease advances. Adjusting for that narrowing window depends heavily on subjective diaries, a tool poorly suited to catching the swings that matter most.

The sweat sensor readings closely matched lab-measured blood levodopa levels in both healthy and Parkinson's subjects after a single immediate-release levodopa/carbidopa dose. Blood pressure data improved the model's accuracy further. Peak levodopa levels coincided with patients' best motor scores, showing this period carried genuine pharmacodynamic meaning.

The patch draws sweat passively through a salt-loaded gel, then runs it across an enzyme sensor that generates its own small voltage, so it needs no battery, no stimulation, and no exertion.

The ultimate larger goal is a closed-loop system that links continuous monitoring to a pump delivering levodopa doses on demand. Validated across repeated real-world use, that would give clinicians something they currently lack: an objective read on and control of levodopa levels between visits rather than relying on a patient's after-the-fact recollection.

"This approach establishes a foundation for real-time, at-home therapeutic optimization and advances the development of future closed-loop treatment systems for PD," the authors conclude.
