After first antipsychotic failure, clozapine may be most effective next step

A randomized trial evaluated 654 patients (ages 16 to 45) with first‑episode psychosis to determine whether switching to clozapine after a single ineffective antipsychotic improves response. In stage two of the trial, among patients who didn’t respond to initial therapy, clozapine achieved the highest response rate (62.5%) compared with amisulpride (44.7%) and olanzapine (31.7%), without meaningful differences in discontinuation across groups.
Clinical takeaway: When a patient with first‑episode psychosis shows no response to their initial antipsychotic, consider earlier transition to clozapine to maximize symptom improvement.
